H. pylori colonization causes persistent inflammatory responses.H. pylori-induced gastritis is the strongest singular risk factor for cancers of the stomach however only a small proportion of infected individuals proceed to the development of malignancy. It is still unknown what factors are responsible for the different outcomes ranging from asymptomatic gastritis, peptic ulcer disease (PUD) to gastric cancer (GC). The i1 variants of have been shown to have stronger vacuolating activity than i2 variants (Jones et al., 2011). Atherton et al. (1995) reported three of the four possible subtypes with the exeption of s2-m1 variant. The presence of the s2-m1 variant was later reported but with very rare prevalence (0 to 3%). Despite variants have been described, but is still remains unclear whether the genotypes are useful markers for the prediction of clinical outcomes. Initial reports indicated tha t the s1 and i2 genotype would be found in close association with clinical outcomes in Western countries (Jafari et al., 2008); however the prevalence of this genotype was extremely high (almost 100%) in East Asian countries irrespective of the clinical outcomes (Jafari et al., 2008). The prevalence of H. pylori infection is about 50% of the world’s population and has been reported to be 40 to 60% in Malaysian (Goh et al., 2009). The purpose of this study is to identify the prevalence of H. pylori isolates in patients with a full spectrum of clinical demonstration of H. pylori infection to evaluate whether the assumed associations with genotypes and cagA gene would be applicable to patients with H. pylori infection in Malaysian populations. 2. Material and methods Isolation and culture of H. pylori Gastric biopsies were obtained from each patient by using endoscopy. Each specimen was placed in a transport medium and sent to the laboratory immediately. The specimens were then incubated onto chocolate (selective/non-selective) agar containing 10% sheep blood for 3 days at 37 ºC under microaerobic conditions (10% CO2, 5% O2, 85% N2). All H. pylori strains were positive for urease, oxidase, catalase and were identified by Gram staining under light microscopy. Colonies directly harvested from the plates were used for DNA extraction. Endoscopic findings were also recorded and based on the information patients were divided into two groups. Group I patients had duodenal or gastric ulcer (Peptic Ulcer Disease, PUD) and group II patients had Non-Ulcer Dyspepsia (NUD). Population studied A total of 209 gastric biopsies were collected from patients presenting with symptoms of gastroduodenal diseases during July 2011 to August 2012 at the University of Malaya Medical Centre (UMMC). Four biopsy specimens were taken from the greater curve of the antrum, body, cardia and Esophagous, one was used for histological examination and one for H. pylori culture. Genomic DNA extraction Bacteria from both the 10 single colonies and the sweeps were resuspended in 1.2 ml Elution buffer (EB). Bacterial pellets were obtained by centrifugation at 5,233 Ãâ€"gfor 5 min, and genomic DNA was extracted by using a Qiagen kit. The amount of DNA was calculated by spectrophotometry. Detection of H. pylori and genotyping of 16sRNA, cag A, E, L and vac genes and EPIYA motifs by PCR In this study PCR was used to detect the H. pylori specific 16sRNA gene for confirmation of H. pylori isolates, the virulence-associated vacA mosaic structure and the presence of cag A, E, L genes. All primer sets were selected from the published literatures (Table 1). The PCR was carried out in a volume of 25  µl including100 ng of genomic DNA, 20 pmol of each of primers (Table 1), 5  µl of i-PCR 5X Master Mix (iDNA, Singapore) which contain 0.1 unit/ µl 5  µl of i-PCR 5X Master Mix (iDNA, Singapore) which contain 0.1 unit/ µl of Taq DNA Polymerase, 1 mM of dNTP’s, 10% of glycerol and 7.5 mM of MgCl2. Negative control tubes included all materials except template DNA. The sequences of the primers are listed in Table 1. PCR products were checked on 2.0% agarose gels with 100 bp ladder size markers. Polymerase chain reaction (PCR) for the EPIYA motifs was performed in separate reactions according to the method described by Argent et al. Loneliness in of Mice and Men :: John Steinbeck

A novel which explores the the theme of loneliness is 'Of Mice and Men' by John Steinbeck. The characters George, Crooks, Candy and Curley's wife are used to show loneliness. In the novel the two main characters, George and Lennie, are starting a new job in California after being ordered out of their last job as a result of Lennie's childish behavior. They work as farm laborers and have always had a dream of they could own their own land. This dream they have is typically an aspiration that many people have but unfortunately never attain. Although, luckily enough they get offered money which by a character called Candy, and this sum of money boosts their chances of capturing their dream. However, the kindled hope they have soon evaporates, with tragic consequences. George's often dismaying desire is to accompany Lennie. Georges loneliness is very peculiar as unlike the other characters who face seclusion, George is united with a friend. However, Lennie is a phenomenally immature adult and as a result feels isolated. This affects him as he ends up either instructing or telling of Lennie when they talk which ultimately means George cannot have adult conversation, something which he deeply desires. An example of Lennie immaturity is shown when George enquires why he has a dead mouse. '' It on'y a mouse. Jus a dead mouse. George. I didn't kill it, honest. I found it dead''. The way Lennie tries to make out there is nothing wrong illustrates his lack of knowledge and insight into the world and shows how this may affect the interaction between the pair. It culminates in the assessment that although George may have a friend, he is so alien to normal individuals that in George's eyes, his human credibility is non-existent. Despite this, he has built a sound relationship with an innocent and nice person who he fears may suffer on his own. For these reasons he finds it difficult to detach himself from Lennie. Nonetheless, his decision to stay ultimately means he will be stuck with a person who he cannot have a grown-up relationship with and have the enjoyable experiences he might have if he was with a more intelligent person .Subsequently George is not able to live the way he wants to live as Lennie acts as a hindrance to his plans. George's reluctance to depart from Lennie's side generates a disservice for himself but at the same time he creates hope for Len nie who couldn't cope by his lonesome. Look on the Bright Side Essay

Throughout everyone’s life they are surrounded by negative people and their thoughts and numerous negative environments. It is your own choice as to whether you are going to see the world in a positive or a negative aspect. As for myself, I chose to try my best to live my life as an optimist. It is not realistic to say that I am an optimist about every single aspect of my life. However, there are many times in particular where I am optimistic about certain situations. This first situation is when I was born I had clubbed feet. In other words, my feet were turned 180 degrees towards each other. The doctors did not think that I could walk, as I grew older. They were determined to do what they could to get me to walk. I was put through 5 surgeries on each foot, put in casts, orthopedic braces all by the time I was 5 years old. The doctors in the end proved my parents, themselves, and myself wrong, because here I am today walking. By having gone through all this I grew up to see the positive sides of many things such as running and sports and much more because I was not expected to be able to participate in these activities. As I grew up, I had a passion for swimming and playing volleyball. I was on the Varsity Volleyball and Varsity Swim Teams in high school, and could not wait until my senior seasons of my favorite sports. Unfortunately, the third week of my volleyball season I was informed that I had fractured my talus and also had a few stress fractures. At that point I had never been more devastated. Through all the doctor appointments and therapy with my schools trainer I held my head high. I still attended every single game, practice and tournament with my team. I still led them in cheers, and poured my heart to them from the bench. It felt like a knife stabbing through the heart watching from the bench, but I had to be them for them, we were family. This experience helped me learn how to take an extremely difficult time period and put an optimistic spin on it. I wanted to be there cheering for my team, I wanted to see the win and losses. I did not want to be upset and depressed about the situation because there was nothing I could do about it, the only thing I could do was cheer until I had no voice. And that’s exactly what I did. The last situation that I have is a current situation that I am dealing with. It is my choice as to where I was going to attend school after I graduated high school. I am attending Northern Virginia Community College. This was not my first choice of schools, and I was extremely upset about this at first. I saw my friends leave Manassas and go their separate ways. They did not live with their parents, and had no super vision. Everything that they had, I wanted. But after church on Sunday I was more of thinking than listening to my priest’s homily. But I realized that I have just as much if not more than they do. I appreciated that my parents were allowing me to live with them and pay for my bills and education. I was grateful that I was able to work and make money while still attending school. From there, I was pleased with the decision of attending NOVA. It was just how I perceived the situation.
